The Unexpected Problems With Plant-Based Diets

Plant based eating gets credited constantly with lowering the risk of heart disease, certain cancers and diabetes, and that reputation is largely well earned. A newer body of research has started raising a more specific, less discussed concern that rarely makes it into the same headlines.
Several large, well designed studies have converged on the same unexpected finding, tied specifically to bone health rather than the usual suspects like protein or B12. The risk is real, but it turns out to be more manageable than the alarming headlines initially suggested.
None of it means plant based eating is secretly dangerous. Here are the unexpected problems researchers keep finding, and what actually explains them.
Bone Fractures Show Up More Than Expected
An analysis from the EPIC Oxford study, one of the largest and longest running studies of vegetarians and vegans, found vegetarians had a 50 percent higher risk of hip fracture compared to regular meat eaters, according to Plant Based Health Professionals.
In practical terms, that works out to roughly 3.2 additional fractures per 1,000 people over a decade, a real but relatively modest increase.
A separate systematic review combining data from two large cohorts totaling more than 76,000 participants found vegetarians faced a 34 percent higher hip fracture risk specifically, with vegans showing the highest overall risk of any group studied, according to ScienceDirect.
Researchers describe the evidence as genuinely elevated risk rather than a statistical fluke.
The Culprit Is Not Just What’s Missing
Part of the explanation traces back to lower calcium, vitamin D, protein and B12 intake commonly seen in plant based diets, all nutrients directly tied to bone strength. Lower body weight among vegetarians in these studies also plays a real role, since less mechanical load on bones over time is independently linked to reduced bone density.
A less discussed factor involves plant compounds called phytates, which naturally block the body from fully absorbing minerals like iron and zinc from otherwise nutrient rich foods. That absorption problem means someone can technically eat enough of a mineral on paper while still falling short of what the body actually manages to use.
Plant Based Meat Alternatives Add Their Own Twist
Switching specifically toward newer plant based meat and dairy alternatives, rather than whole foods like beans and lentils, appears to introduce its own distinct problem.
Researchers found that diets built around these products showed significantly lower vitamin B12, calcium, potassium, magnesium and zinc than diets built around whole plant foods, according to a review published on PMC.
That distinction matters enormously, since a plant based diet built around processed substitutes is nutritionally quite different from one built around lentils, tofu and leafy greens.
A newer systematic review has specifically linked strict plant based eating to a higher combined risk of both anemia and osteoporosis when these gaps go unaddressed, according to Cureus.
None of this means plant based eating is a bad choice, and researchers are careful to note that sufficient calcium and vitamin D intake appears to close most of the fracture risk gap entirely. It just means the diet, like any other, needs to be actually planned rather than assumed to be automatically complete.
