Zac Efron’s Baywatch physique became one of the most talked about transformations in recent Hollywood memory. What rarely got discussed at the time was exactly what it took to get there, or what it cost him afterward.

What The Baywatch Body Actually Required
For the entire Baywatch shoot, Efron followed a rigid eating plan built around clean fish or meat paired with sweet potatoes, repeated at the same three meals every single day. When asked about the toughest diet he has ever done for a role, he answered bluntly, probably just not eating.
The eating plan was only part of it. Efron also relied on powerful diuretics to strip water from under his skin, a method that creates the kind of hyper defined look often associated with bodybuilding stages rather than everyday fitness.
“That ‘Baywatch’ look, I don’t know if that’s really attainable,” Efron told Men’s Health. He has said the effect required so little water retention in the skin that it looked almost artificial on camera.
The Toll It Took Behind The Scenes
The physical routine came paired with an unsustainable schedule, including early morning workouts regardless of how late filming ran the night before. Efron told Variety he was running on very little sleep for months at a stretch, sometimes as little as four hours a night.
The combination eventually caught up with him. He has described developing insomnia and falling into a lasting depression once filming wrapped, a period IndieWire reported took him roughly six months to recover from.
Efron has attributed much of that fallout to the diuretics specifically, saying doctors ultimately linked his symptoms to using them for far too long. It was, by his own account, one of the harder stretches of his career, physically and mentally.
Why He Walked Away From It For Good
Since then, Efron has been open about no longer chasing that level of leanness for any role. He has said he now prefers carrying a bit more body fat if it means avoiding the extremes that came with Baywatch.
His daily habits have shifted too, leaning more on stretching, yoga and recovery work rather than the punishing training schedule of a decade ago. He has also spoken about choosing his own wellbeing over chasing a specific look on camera.
That shift reflects a broader conversation happening across Hollywood about the real cost of extreme physical transformations. Efron has become one of the more visible examples of an actor willing to say publicly that a famous body was not actually worth what it took to get there.
The Baywatch physique may still show up on best body lists, but Efron himself has made clear he sees it differently now. What looked like peak fitness from the outside was, by his own account, one of the least healthy stretches of his life.
